Metabolic Dysfunction-Associated Steatotic Liver Disease (MASLD), previously known as non-alcoholic fatty liver disease (NAFLD), persists as a severe health concern. This condition arises when fat accumulates in the liver, often due to metabolic disorders such as type 2 diabetes or obesity.
Research has shown that MASLD can progress to more severe complications, such as metabolic dysfunction-associated steatohepatitis (MASH), which can lead to cirrhosis or significant liver fibrosis – the development of scar tissue in the liver. Left unmanaged, MASLD can result in deteriorating liver function or liver cancer, diabetes-related complications, and a higher risk of death from various causes, not limited to liver-related conditions.
In a recent study, researchers at the Karolinska Institutet in Sweden analyzed the risk of death in 13,099 MASLD patients compared to the general population. Their findings revealed an alarming trend: those with MASLD had a twice as high mortality rate overall, with a 27-fold increase in the risk of liver disease and 35-fold increase in liver cancer mortality. Interestingly, the most common causes of death for these patients were cardiovascular disease and non-liver cancer, respectively accounting for 54% and 47% higher mortality risks.
Study lead Axel Wester, an assistant professor at the Karolinska Institutet and physician at Karolinska University Hospital, highlights the need for early intervention and a holistic approach. "Many people are not aware that they have fatty liver disease because it rarely causes symptoms in the early stages," says Wester. "Our study shows that people diagnosed with MASLD have an increased risk of dying from many different diseases, not just liver disease."
The study further found that MASLD patients had an elevated risk of death from infectious diseases, gastrointestinal diseases, respiratory diseases, endocrine diseases, or external causes.
To minimize the risk of developing MASLD or experiencing its severe complications, medical experts recommend the following preventive measures:
- Weight loss: Aim for at least a 10% weight loss if you are obese, or a 3 to 5% reduction even if you only need to shed a modest amount of weight.
- Regular exercise: An exercise regimen enhances Kupffer cells' protective function for the liver in addition to aiding weight loss.

In the early stages, MASLD typically causes no noticeable symptoms. When symptoms do appear, they may include vague complaints like fatigue, discomfort in the upper right abdomen, unexplained weight loss, muscle weakness, and edema. More pronounced symptoms don't emerge until the disease has progressed, such as confusion, behavioral changes, jaundice, easy bruising, itching, nausea, loss of appetite, and visible spider-like blood vessels on the skin. Early detection in high-risk individuals (such as those with obesity or diabetes) through blood tests and imaging is crucial for early diagnosis and successful management of MASLD.
